Please submit your referral through Epic. If you do not have Epic, complete one of the following referral forms and submit it via fax.
General Referral Form
Complete this form to refer a patient to any Enloe Health service except for our cancer services. Please submit the form via fax if you do not have Epic. Get the fax number for the desired service from the Referral Directory.
Cancer services at Enloe Health require different referral forms. Find our cancer referral forms below.
Cancer Referral Form
To refer a patient to Enloe Health Hematology Oncology, Enloe Health Infusion Therapy, Enloe Health Genetics or any other cancer service at Enloe Health, complete this form. Please submit the completed form via fax if you do not have Epic. Get the fax number for the desired service from the Referral Directory.
