If an out-of-network provider does not accept insurance billing, you can submit a claim for reimbursement under your Enloe Health plan by uploading your claim using myluminarehealth.com:
When submitting a claim, it is important that all necessary information is provided. This includes:
- Patient name
- Dates of service
- Provider name and details
- Total amount charged for the service
- Copies of receipts or itemized superbill
