Who will make decisions about your care if you are not able to speak for yourself in a medical emergency? Do the people closest to you know what kind of medical care you want — and don’t want?
All those 18 and older should document who they want to be their decisionmakers, along with what kind of medical care is desired in the case of emergency. Attend this 90-minute workshop to begin the process of advance care planning (ACP). You’ll:
- Learn the five easy steps of ACP.
- Understand the different ACP documents like the advance directive and POLST (Physician Orders for Life-Sustaining Treatment).
- Get the documents you need.
- Ask questions and receive answers.
- Complete a worksheet of next steps.
- Play the “Go Wish” game!
Please note that every workshop covers the same material.
