Transitional Care

After leaving the hospital, it can be hard to see your primary care provider right away. This delay can make it confusing to keep track of your medicines and care plan.
The Enloe Health & Wellness Transitional Care team helps by giving you timely follow-up and support, so you feel confident moving forward.
Our caregivers will review the following with you:
- Discharge instructions
- Your medications
- Questions related to your admission

What to Expect
Before your first appointment with our Transitional Care team, a member of the team will call you after you leave the hospital.

During this call, the caregiver will connect with you about your recent admission and expectations for your upcoming appointment, including resources you might need to get to your appointment.
Make sure to bring the following to your appointment:
- All your medications, including new prescriptions and your pill case
- Your blood pressure monitor
- A glucometer, a device that tests your blood sugar
- Bring the person who truly supports your health, such as the one who helps manage your medications, appointments or care
- Any logs you use that track your blood pressure, blood sugar, weight and or symptoms
What is Transitional Care?
Transitional Care is a service that supports you after a hospital stay. Our caregivers support your transition back to the community and your primary care provider.
When someone leaves the hospital, there are often new medications, instructions and follow-up needs. Our team steps in to help you navigate these aspects of your care. We act as a bridge between the hospital and your primary care provider, creating a smooth transition back to ongoing care.
Prioritizing Our Patients’ Care
Our goal is to keep you on track and reduce your chances of another hospital stay. We provide short-term support, typically 30 days or less, while you transition from in-hospital care back to your primary care provider.
During this time, we coordinate tests, referrals, and results, so your provider stays informed, and you have clear guidance. Transitional Care connects your hospital experience to your everyday life, creating a smooth path back to your health team and community.
