Discharge Planning & Referrals

Happy woman in the park with husbandOur goal at Enloe Behavioral Health is to help you manage your mental illness by teaching you how to recognize triggers, understand the importance of managing your medications and helping you begin the process of dealing with day-to-day life.

As soon as you’re admitted, your care team will start working on discharge planning. You’ll meet with a trusted social worker, who will help plan your care once you leave our facility.

Social workers will help you find an in-network therapist and a psychiatrist you can visit after your stay with us. Depending on your needs and wishes, the social worker may also:

  • Connect you with government and county resources
  • Help you find support groups
  • Identify useful parenting classes
  • Help you find places of worship
  • And more
