Reviewing Your Record During Your Stay
We want you to play an active role in your health care. At Enloe Medical
Center, we encourage you to read your medical record during your hospital
stay to be better informed about your health and the care you receive.
Simply let your nurse or doctor know that you would like to review your
medical record, and he or she will arrange a time to review it with you.
Your privacy is important to us. If you would like someone in addition
to yourself to have access to your medical record, you will be asked to
sign an authorization form.
Obtaining Your Record After Discharge
To receive a copy of your medical record to keep, please contact the
Medical Records Department two to three business days after you
have been discharged from the hospital to allow time for your record
to be completed and processed. You will be asked to complete an
authorization form and show a picture ID to receive copies or your
medical record. You may obtain
the authorization form.

The Medical Records Department
is located at 1531 Esplanade, in the
basement of the hospital, and its
hours are as follows: Monday through
Thursday, 8 a.m. to 4 p.m. and Friday
8 a.m. to 2 p.m. Closed on holidays.
The first 20 pages you receive are
complimentary. You may receive
these records on paper or CD.
